For the child or adolescent who has serious emotional problems, complex needs and requires the assistance of a number of agencies or systems. Crossroads provides an array of intensive services designed to help keep families intact and working well together. Families are the major influence on a child's health and development and they are the most important support system for children with special health needs. Our staff delivers family-centered services, and works to promote, support and empower youth and their families through parent-professional partnerships.

Community Support Program ...

Primary Community Support workers identify, develop and coordinate formal and informal resources in partnership with families, and assist in accessing other intensive mental health services. They work with children and families in the home and community utilizing family strengths. As part of the team, they help put together flexible, creative and individualized plans in response to needs. Community Support Program services are provided on an individual basis and in groups.

Partial Hospitalization ...

For children who have severe impairments in social, emotional and academic functioning, a daily setting which is highly structured and supports each child's individualized need is sometimes required. Crossroads Partial Hospitalization Program provides a therapeutic environment, including art and recreational therapy along with individual and group counseling, joined with a certified academic classroom.

Our skilled multi-disciplinary team uses a level achievement behavioral approach to positive change with the objective of returning the child to a regular classroom and less intense mental health services as quickly as possible.

Respite / Therapeutic Foster Care...

Crossroads maintains a small network of therapeutic foster homes for children who require a time limited out of home placement. Placement is generally utilized to give the child and family an opportunity to address issues that exist between them, without the stress of continuing to live together. Children placed in this program remain in the custody of their parents. During the placement, child and family participate in intensive treatment with the goal of returning the child home as soon as possible.

Respite care is designed to provide families and children having emotional problems with some planned or emergency time away from each other to re-group, resolve problems and maintain coping abilities. Respite can be provided for a few hours, or overnight, depending on needs and provider availability.
